However, they offer a unique opportunity to raise the standard of care at
institutions, while at the same time to include all the appropriate tests and
treatments for patients. In this fashion, they serve as a readily available
guide to the clinician when treating the patient. They also may serve as a means
of improving compliance with current guidelines for treating patients.
We are delighted to present in this book a full array of critical pathways from
leading institutions around the country. These pathways are the best practices
for these hospitals, developed by leaders in their areas of expertise. The
authors have done a superb job of integrating the key clinical data into brief
summaries and then providing the critical pathways that they use at their
hospitals. We have organized this book in a fashion to facilitate using the
information. We begin with an overview of what critical pathways are and how to
develop them. We then present several pathways for management of patients in the
Emergency Department, focusing on patients with acute coronary syndromes. The
next section covers the types of patients who are admitted to the hospital, and
the final section includes several outpatient critical pathways, such as
management of hypertension and lipid disorders. It is hoped that these pathways
will allow clinicians around the world to adopt critical pathways more easily as
a means of improving the care of their cardiac patients.

The 37 chapters in Current Diagnosis & Treatment in Cardiology cover the major
disease entities and therapeutic challenges in cardiology. There are chapters on
major management issues in cardiology such as pregnancy and heart disease, the
use of anticoagulants in heart disease, and the pre-operative evaluation of
heart disease patients. Each section is written by experts in the particular
area, but has been extensively edited to insure a consistent approach throughout
the book and the kind of readability found in single-author texts.
Since the first edition the book has grown somewhat. Each chapter has been
thoroughly revised and the references updated. Also, there are three new
chapters in areas that have increased in importance in the last 5 years:
coronary revascularization, atrial fibrillation, and aortic aneurysms.
My hope is that the book is found useful and improves patient care. Also, I hope
it is an educational tool that improves knowledge of cardiac diseases. Finally,
I hope it stimulates clinical research in areas where our knowledge is
incomplete.

It has been nearly 30 years since the first edition of Urologic Surgery was
published in 1969. Since then, the field of urology has undergone massive
transformation. We have witnessed advances and improvements in virtually every
aspect of our surgical craft. An exhaustive survey of improved capabilities
would include reconstructive techniques, endourology, laparoscopic surgery,
mastery of multiple energy sources for transurethral surgery, and tremendous
strides in pediatric urology.
However, certain fundamentals remain constant. As stated in the preface to the
first edition, Urologists arefirst and foremostsurgeons. The second edition
expressed the hope that the volume constitute the basis for further advances
and that it be rendered obsolete by progress in urology. In the third edition,
it was acknowledged that progress in urology was paralleled by advances in
anesthesia, antibiosis, medical techniques, and diagnostic capability. The
fourth edition reaffirmed that although many textbooks of urology and a number
of excellent atlases dealt with surgical procedures, no single volume combined
the virtues of text and illustrations that amplify the fundamental
considerations and technical aspects of urologic surgery.
In retrospect, these thoughts are verities. We are still seeing rapid advances
in our specialty. Perhaps there is no greater compliment to a medical
publication than to admit to obsolesence in only a matter of a few months or
years, as the result of our expanding capabilities. However, it is our expressed
hope that this volume will serve as a ready reference for medical students,
residents in training, and even our colleagues with the most advanced surgical
skills.
On a personal note, it is with great satisfaction that the editorship of
Urologic Surgery passes on to Sam D. Graham, Jr., M.D., my capable friend,
respected colleague, and former resident. He has assembled an outstanding group
of authors for the fifth edition, and I anticipate with pleasure the prospect of
further editions.
